Complete and accurate medical forms help us ensure maximum safety and care for each camper. The Westchester County Department of Health monitors compliance with the state’s sanitary code for all Westchester camps, and requires that these forms contain complete and up-to-date information. NO CAMPER WILL BE PERMITTED TO START CAMP WITHOUT A COMPLETED MEDICAL FORM, INCLUDING IMMUNIZATIONS AND SIGNATURES, ON FILE.
- 2010 Camper Medical - This is a two-side form and should be completed and signed by a parent. The back is for your physician, although he or she may substitute his/her own form as long as it bears the physician’s signature. Up to date immunizations must accompany the form.*
- 2010 Parent Medication Consent Form - If your child requires prescription medication(s) to be administered during the camp day, this form must be completed and signed by a parent and the camper's physician, one form for each medication to be administered.
- 2010 Epi-Pen Information and Agreement - If your child requires an epi-pen, this form must be completed and signed by a parent and the camper's physician.
- 2010 Epi-Pen Bus Form - In addition to the epi-pen held at camp, you may authorize another epi-pen to be transported with your camper on the bus by completing and signing this form.
